When people say “I can’t hear myself think”, it means that there is so much noise their brain spends all the time listening to what’s happening or getting distracted by what’s happening around them and they can’t concentrate on what they’re thinking about. Sometimes you imagine your voice when you’re thinking about what to say, or imagine music, but you can’t hear your thoughts with your ears.

Sometimes you might have a conversation with yourself if you’re considering a hard choice or if you’re trying to figure something out. When you do that, you’re basically approaching a problem from several different angles and you kind of speak your thoughts to yourself. That’s the closest (for me anyway) that you can come to actually hearing yourself think.
